We invite you to join our team!-:What You'll Do:As a Case Manager, your advocacy will ensure our short-term, skilled nursing patients are medically stable with a safe discharge plan to promote success in the community and limit readmissions.Key Responsibilities:Advocate for residents' rights and preferencesReview clinical documentation to define expectations, criteria, and variables that will impact the care plan and outcomesCollaborate with treatment team to create a patient-centered case of medical necessity and rationale for skilled nursing coverageProvide rationale and supporting documentation regarding continuum of care to promote best practice outcomes and patient-centered discharge planningDevelop and maintain relationships with insurance representatives and facilitate managed care team meetingsIf you are a compassionate and organized healthcare professional with a passion for ensuring positive patient outcomes, consider joining our team as a Case Manager!
