Job Description:
A Magnet designated acute care hospital located in central
Connecticut is currently searching for an experienced Registered
Nurse RN Case Manager to join their award-winning healthcare
team.For more than a century, this leading healthcare organization
has provided world-class acute care services to the surrounding
communities. Known as a national leader in healthcare, this
facility boasts 150+ inpatient beds including a state-of-the-art
emergency department that treats nearly 50,000 patients each year.
The main campus includes an award-wining intensive care unit,
single room model maternity units, wound care and hyperbaric
center, a leading skilled nursing facility, and an established
surgical center.Reporting to the Director of Case Management, the
RN Case Manager will provide clinically-based care management,
utilization review, and discharge planning services. The RN Case
Manager will partner experienced Social Workers and other
healthcare professionals to identify appropriate utilization of
resources, ensure timely and accurate reimbursement, create
comprehensive discharge plans in accordance to patient needs, and
educate both patients and families.Requirements Include:Bachelor's
degree in Nursing BSNCurrent Registered Nursing licensure in state
of ConnecticutPrior experience in acute care case management or
utilization reviewNursing experience in a Medical-Surgical setting
