Job Description:
*Case Manager, Registered Nurse*In this role, the Case Manager,
RN assists with services that help the patient gain access to
needed medical, social, educational, and other services. This
includes direct assistance in gaining access to services,
coordination of care, oversight of the entire case, and linkage to
appropriate services. Case Managers assess the patient in determine
service needs; develop an individualized integrated care plan,
refer and coordinate activities to help the patient obtain needed
services and effective discharge planning.*Requirements:** NYS
Registered Nurse license* BSN required* Minimum of 1-2 years of
clinical experience in nursing; preferably in hospital case
management.* Experience with InterQual and Milliman Care
Guidelines.* Knowledge in DRGs and GMLOS* Managed Care and
Regulatory Requirements preferred.* In addition, knowledge of
regulatory guidelines governing hospitals. Medicare, Medicaid,
Island Peer Review Organization (IPRO) and other Third Party
criteria and standards as applicable.The employee will be a member
