Job Description:
JOB PURPOSE:The SCY Community Health Worker (CHW) provides
outreach, screening, advocacy, and barrier intervention for
families of children referred to the Screening Connecticut's Youth
program for identification of at-risk mental health indicators and
referral to behavioral health interventions. The CHW ensures the
coordinated access to community-based supports and behavioral
health treatment resources to meet identified client needs.General

and other evidence-based models.Demonstrated advanced computer

utilization of mobile devices.Knowledge of comprehensive community
and state services available within the region that provide
behavioral health services and support to overcome barriers to
mental health services for families of various cultural and
economic backgrounds.Skilled in crisis management/intervention with
individuals and families, and familiarity with the related
regional/state systems and services.Skilled in generating and
maintaining relationships with other providers for coordinated and
seamless service delivery.Knowledge of systems and conditions
impacting clients' ability to achieve well-being, including factors
of race, culture, and equity.Demonstrates professional, courteous,
and respectful attitude during interactions with patients and
guests of the health center.JOB EXPECTATIONS:Provides culturally
and linguistically appropriate case management services to
clients.Complies with SCY contractual obligations and
outcomes.Follows and implements all SCY policies and
procedures.Represents SCY CM program professionally within the
program and in the community.Meets regularly with CCFSA SCY Program
Manager for supervision, support, and guidance regarding cases in
ETO on a daily, weekly, or bi-weekly basis (atManager's
discretion).Remains current with professional credentialing
requirements of self and Community Health Workers and professional
best practices.Participates in CCFSA meetings and activities and/or
professional group seminars, conferences, activities to increase
knowledge and accepts responsibility for professional
development.Maintains flexible work schedule to accommodate CCFSA,
program and client needs, including non-traditional hours as
needed.SPECIFIC DUTIES and RESPONSIBILITIES:Conduct SCY program
outreach with all referred families.Administer approved SCY
screening tools to all consenting program participants.Schedule
mental health intakes for eligible, consenting participants.Verify
attendance at mental health intake.Identify and mitigate barriers
to participant attendance at mental health appointments.Perform
patient navigation and facilitate patient/client access to
appropriate mental health providers and social services resources
internally within the agency and externally in the
region/state.Educate patient/client and family/caregiver(s) about
relevant community resources.Request and verify all demographic
information.Administer post-preventative surveys.Collaborate with
internal and external agencies resources to meet patients'
needs.Conducts community outreach events and community
collaborations to assist with referrals.Attend all program and
skill development training courses/webinars and meetings assigned
or approved by Program Manager.Coordinates crisis intervention
efforts for clients and assists with CHW referrals to
community-based, emergency behavioral health services and
supports.Utilizes the Efforts to Outcomes (ETO) web-based data
management system to inform management and supervision of CHW daily
operations, including documentation of pertinent information on
client interactions in a timely and proficient mannerProvide
program feedback to Program Manager to continually improve
program.Performs other reasonable duties as requested by CCFSA SCY
Program Manager or other CCFSA leadership team members.EDUCATION
REQUIREMENTS:Minimum of a High School degree with relevant
experience in the field of healthcare, behavioral health, or human
services as an Outreach Worker, Case Manager, Lay Health Worker,
Peer Educator, Community Health Advocate, or equivalent. Additional
college education (Associates or Bachelors) a plus.Community Health
Worker Certification from Connecticut-approved provider is
preferred.
