time where Geriatric & Palliative Care is in great need, Hartford
HealthCare is leading the way to provide access to Connecticut
residents. We are seeking a Nurse Practitioner or Physician
Assistant to join our growing outpatient Palliative Care team in
Southington, CT. The APP will work in an outpatient setting
alongside a Geriatrics provider, with strong collaboration from a
regional team that includes inpatient Palliative Care physicians
and APPs as well as Geriatric Medicine colleagues. This is an
exciting opportunity to be part of a rapidly expanding Palliative
Care program within a supportive and innovative system. Experience
or interest in home-based palliative care is a plus! Role : Open to
Nurse Practitioners (APRN) and Physician Assistants (PA) Location :
Southington, CT Setting : 100% Outpatient Palliative Care Schedule

Nurse Practitioners and Physician Assistants Role Scope: Under
established guidelines and in collaboration and/or supervision with
the practice physician(s), Nurse Practitioners and Physician
Assistants perform routine physical examinations, and formulate
medical, educational, and psychosocial plans to treat patients. In
consultation with the physician(s) and care team members, Nurse
Practitioners and Physician Assistants develop plans for further
evaluation of health needs, e.g., referral to other health team
members or specialists. Nurse Practitioners or Physician Assistants
provide appropriate treatment for problems within the scope of the
provider’s practice as designated by protocols and identifies
problems outside of their scope of practice and refers
appropriately to physician. Qualifications Requirements : Licensure
: Unrestricted State of Connecticut Medical License (or eligible)
Training : APRN (Family, Adult/Geri Nurse Practitioner) or PA
Experience : 2 years’ experience in a palliative care and/or
