Job Summary The Physical Therapist is responsible for evaluating physical therapy needs and delivering comprehensive, medically prescribed treatment to patients in a supervised rehabilitation setting. The role includes developing individualized treatment plans based on patient capacity and setting realistic, achievable goals. Responsibilities Document and record patient conditions and provide education to patients and families regarding physical therapy plans. Perform initial assessments and evaluations for designated patient populations. Review medical charts to obtain relevant clinical information and clarify referrals when needed. Select appropriate assessment tools and techniques to accurately determine patient needs and create optimal treatment plans. Request treatment orders when needed for unfamiliar diagnoses. Develop and implement discharge plans in collaboration with patients, caregivers, and care team members.
