Location Detail: 300 Queen Street Southington (10120) Shift Detail: Occasional on call, may be expected to fill in every other weekend. Central region in general, cross train to cover GIP Hospice Work where every moment matters. Our Hospice Visiting Nurse (RN) utilize their extensive education and training to provide personalized care to our patients in a rewarding environment that promotes autonomy. Job Description - Support our patients, families, and loved ones, as they go through their end of life journey - Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies - Be a member of an active interdisciplinary team that addresses patients’ needs holistically - Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families. - Utilize state-of-the-art technology to document skilled services provided. Qualifications - Registered Nurse with Valid CT RN license - One year of RN experience; Homecare or Hospice experience preferred. - Valid driver’s license and reliable transportation required. - Computer proficiency required.
