the Position TravelNurseSource is working with Cynet Health to find
a qualified Home Health RN in Rutland, Vermont, 05701! Job Title:

Experience: Minimum 2 years License: Active Nursing License
Certifications: OASIS experience required Must-Have: - OASIS
experience Description: Utilizing the nursing process, assesses
patient/caregiver needs to provide quality care to assigned
patients in accordance with evidence-based practice and State and
Federal regulations. Develops and communicates a realistic plan of
care for each patient. Ensures that the patient and caregivers are
included in the care plan process. Serves as Case Manager for
assigned patients and actively manages caseloads and visit times.
Effectively fills out and completes OASIS documentation in
accordance with agency policies and procedures. Demonstrates
knowledge of, and keeps up-to-date on changes to OASIS
documentation requirements. Implements medical plan of care,
nursing interventions, and procedures as necessary to ensure safe
and effective quality of care. Stays up-to-date on a variety of
high-tech skills, including IV use and maintenance, wound VAC, and
complex wound care. Takes action to ensure that infection control
standards are maintained throughout the plan of care. Works
collaboratively with LPNs to ensure that the plan of care is
followed. Delegates tasks and ensures that patient needs are met
consistently. Evaluates aspects of the implemented plan of care
including identified problems and patient responses. Maintains
up-to-date and accurate documentation of nursing care provided.
Follows agency protocol with documentation processes including
locking down visits and follow-up notes. Performs patient and
caregiver education to ensure informed patient decisions can be
made. Identifies interdisciplinary needs of the patient and
documents accordingly. Transmits patient incident reports in a
timely manner. Coordinates safe patient discharges and/or transfers
and ensures all appropriate paperwork is processed. Coordinates a
wide variety of patient care with physicians, team members, and
vendors. Participates in monthly peer review audits and provides
constructive feedback. Proactively schedules and facilitates case
conferences to maximize communication with team members. Advocates
for patient care issues to ensure overall quality of care. Actively
participates in meetings to develop an appropriate individualized
plan of care. Stays up-to-date on pain control, symptom management,
and high-tech skills. Performs patient and caregiver education to
ensure informed decision-making. Coordinates patient care with
various providers in support of the recommended plan of care. 5
