Reports to: Medical DirectorDivision/Department:Adult Services Programs: VariousJob Code: APRN Job Grade: 25TitlesReporting Into:N/A FLSA Exemption - Check Only One:ExemptNon-ExemptWork Location: 233 Main ST New Britain, CT EEO Category: ProfessionalSummary:Ensures that high quality/best practice psychiatric and medical care is provided to all agency clients.Collaborates with program teams of professionals and para-professionals to identify client needsand contributes to the development of treatment plans while supervising client pharmacology perscheduled appointments and/or by being on-call to programs.Essential Responsibilities and Target Outcomes:--- Provides and documents in the medical record psychiatric and pharmacological assessment ofclients in accordance with agency policy and procedure through scheduled and emergencyappointments.--- Collaborates with program staff and provides timely response to client needs for medicationand/or referral and admission to higher or lower levels of care through scheduled appointmentsor by being on-call.--- Actively participates in team meetings, program evaluation, risk management and qualityimprovement functions in consultation with the Chief Clinical Operator and program staff byparticipating in various committee meetings/work groups.--- All other duties as assigned.Principal Customer:DMHAS Target Population of adult and/or child and adolescent clients from from across the State ofConnecticut and their families. All clients referred for substance abuse services as managed byMedicaid and privately contracted payers.Critical Work Relationships:All clinical and medical staff of DMHAS referring facilities/agencies. Regulatory/Accreditation/Payerpersonnel. All providers of ancillary services to agency clients. All staff of the agency affiliatenetwork as well as all agency employees.Community Mental Health Affiliates - Job DescriptionJob Description: APRNEducation: APRN degree from an accredited Nursing school/program with specialty inrelated psychiatric field.Credentials andLicenses:--- APRN licensure in the State of Connecticut.--- Certification and/or eligibility for certification in psychiatricprescribing for appropriate for client population.--- Ability to Prescribe MAT preferred--- Valid Connecticut Driver's License.Experience: --- 1-3 years post degree experience in the provision of psychiatricassessment and medication evaluation of the full range of psychiatricdisorders as identified in the DSM-IV-TR.--- Intermediate to advanced experience with Microsoft Office productsincluding Word, Outlook, Excel and PowerPoint.--- Experience in or the ability and willingness to learn how to use anElectronic Medical Record (EMR) system including: appointmentscheduling, authorization tracking, assessments, treatment plans,progress notes and discharge summaries.Competencies:Behavioral Skill Sets--- Ability to accurately assess and prescribe for the full range of child,adolescent and adult mental disorders as identified in the DSM-IV-TR.--- Familiarity of best practice pharmacology for various mental healthdisorders.--- Understanding of age appropriate developmental milestones for thefull range of child, adolescent and adult populations.--- Familiarity with regulations/standards of care pertaining to MedicalManagement of clients as outlined by JCAHO, DCF and/or DPH.--- Ability to multi-task while setting appropriate professionalboundaries and priorities in routine and emergency situations.--- Ability to ensure the security and confidentiality of client informationand records in a manner consistent with professional and agencycode of ethics.--- Awareness/sensitivity to all types of issues related to client diversity,including cultural, ethnic, language, gender, sexual orientation,gender preference, physical handicap and spiritual diversity.Disclaimer:Community Mental Health Affiliates reserves the right to change or assign other duties andresponsibilities to this position.
