Job Description:
As a Case Manager at {EMPLOYER-DESC}, you will be instrumental
in providing support and guidance to individuals and families in
need. Your role will involve assessing client needs, developing,
implementing, and reviewing service plans, and working
collaboratively with various social services providers to ensure
the well-being of your clients. Your efforts will directly
contribute to our mission of supporting and empowering our
community through dedicated care and personalized service plans.
Conduct comprehensive assessments of client needs and
circumstances. Develop and implement personalized care plans in
collaboration with clients and other professionals. Monitor and
evaluate the effectiveness of service plans, making adjustments as
necessary. Maintain accurate and timely documentation of case
notes, client interactions, and service plans. Provide crisis
intervention as needed and refer clients to appropriate support
services. Advocate on behalf of clients to secure necessary
services and support. Coordinate with other social services,
healthcare providers, and community resources to streamline access
to services. Strong interpersonal and communication skills Ability
to manage multiple cases with attention to detail and
confidentiality Proficient in case management software and record
keeping Strong problem-solving skills and the ability to think
critically Empathetic and compassionate approach with a deep
understanding of social service practices Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Certified Case
Manager (CCM) preferred Licensed Clinical Social Worker (LCSW) or
equivalent state certification is beneficial
