Job Description:
This direct support position assists individuals with
intellectual and developmental disabilities to build and maintain
their ability to participate in community activities by focusing on
skill areas that include communication, self-care,
relationship-building, and community involvement in our Easton, MA
location. As a Community Based Day Supports Coordinator – Level I
you will work in concert with each assigned program participant and
AGB Team members to develop and implement their Person Centered
Plan (PCP) and their Individual Support Plan (ISP).
