Registered Nurse (RN) | Home Health Location: Rutland, VT Agency: Cynet Health Pay: $2,766 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Home Health RN Profession: Nursing Specialty: Home Health RN Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM to 4:00 PM, with some weekends and on-call required Experience: Minimum 2 years required License: Licensed Registered Nurse Certifications: OASIS experience required Must-Have: - OASIS experience - Strong nursing assessment skills - Excellent communication and documentation skills - Knowledge of infection control standards Description: Utilizing the nursing process, assesses patient and caregiver needs to provide quality care to assigned patients, following evidence-based practice and applicable regulations. Develops and communicates a realistic plan of care for each patient in accordance with evidence-based practice and relevant regulations. Serves as case manager for assigned patients, actively managing case loads and visit times in accordance with agency productivity standards. Effectively completes OASIS documentation in line with agency policies and state and federal regulations, demonstrating knowledge of any changes to documentation requirements. Implements medical plan of care, nursing interventions, and procedures to ensure safe and effective quality care for patients. Stays current on a variety of high-tech skills, including IV use and maintenance, wound VAC, and complex wound care. Works collaboratively to ensure that the plan of care is followed, delegates tasks, and ensures that patient needs are met. Consistently reviews documentation, annotates visit notes, and acts on recommendations per department policies and procedures. Evaluates various aspects of the implemented plan of care, including identified problems, care provided, and patient responses. Maintains accurate documentation of nursing care to ensure effective integration of information for use by the healthcare team. Follows agency protocol with documentation processes, ensuring timely completion of required forms. Educates patients and caregivers to ensure informed decision-making regarding treatment options, medication side effects, and other essential information. Identifies interdisciplinary needs of patients and documents these needs accordingly, communicating information to relevant disciplines in a timely manner. Completes various agency, state, and federal forms accurately and in a timely manner to facilitate patient care processes. Conducts visits efficiently, ensuring that supervisory visits are not combined with routine visits when necessary. Assesses patient needs for home care assistance services, developing and coordinating plans of care with patients and caregivers. Transmits incident reports to appropriate specialists in a timely manner as necessary. Coordinates safe patient discharges and transfers, ensuring appropriate paperwork and patient information is processed. Collaborates with physicians, providers, team members, and vendors to support the recommended plan of care for patients and families. Participates in peer review audits, providing and receiving constructive feedback in a professional manner. Schedules and facilitates case conferences to maximize effective communication among team members. Advocates for patient care issues to ensure quality care that is sensitive to each specific patient's and family's needs. Participates in interdisciplinary team meetings to develop an individualized plan of care within specified timeframes. Stays current on pain control, symptom management, and high-tech skills, ensuring infection control standards are maintained throughout the plan of care.
